Dear Doctor,
I hope you are doing well.
I would like to discuss my long-term treatment and ask a few questions about my future.
I was diagnosed with mania (Bipolar I Disorder) in 2023 and have been taking my medications regularly as prescribed. I have an MS in Software Engineering and work as an online teacher.
Alhamdulillah, I am doing very well now. I have no sleep problems, no persistent low mood, no hallucinations, and no significant side effects from my medications. After my fathers passing, I was emotionally affected for some time, but I recovered well and have remained stable.
Before my diagnosis, my condition gradually worsened over time until I eventually lost control. I experienced symptoms including:
- Mania
- Hallucinations
- Severe sleep disturbance (very little or no sleep)
- Suspiciousness and paranoia
- Delusional thoughts
- Extreme irritability and intense anger
- Feeling uncomfortable or overwhelmed in crowded places and by loud noises
- Poor judgment and loss of control
- Difficulty concentrating and being productive
- Confused and disorganized thinking
Because of what I went through, I have no intention of stopping my medication on my own. I understand how important these medicines are in preventing relapse, and I will continue taking them as advised.
My treatment is being managed by Dr. Urooj Tabbassum at Dr. A. Q. Khan, Dow University, Karachi.
I would be grateful if you could guide me on the following:
1. How long am I likely to need these medications? Is there a usual maximum duration for maintenance treatment after a manic episode?
2. If I continue to remain stable, is it possible to gradually reduce the dose under medical supervision, or do some patients need lifelong treatment?
3. I am planning to get married in the future. Will these medications affect my married life, fertility, or pregnancy? How are they usually managed during pregnancy?
4. Are there any long-term side effects of taking these medications for several years that I should be aware of?
5. Since I have been stable for a long time, does this indicate that my condition is well controlled?
