I am a 20-year-old woman seeking medical advice regarding my menstrual changes and pelvic symptoms.
For the past 5–6 menstrual cycles, I have noticed a change in my periods. Earlier, my cycles were irregular and could go up to 40 days, but in the last two cycles they have become more regular, lasting 29 and 32 days.
My bleeding pattern has also changed. Previously my periods were heavier for around 4 days, but now the flow is mainly concentrated in the first 2 days. After that, the bleeding becomes very light and I only change pads for hygiene purposes.
On the 4th day of my period, I regularly pass painful blood clots that are usually grape-sized and sometimes coin-sized. Before passing the clot, I experience strong lower abdominal cramps and pain, which improves after the clot passes.
I also have constant mild lower abdominal/pelvic pain even outside my periods.
On the first day of menstruation, I get severe cramps that significantly interrupt my daily activities, and I often require up to 2 painkillers to get some relief.
In addition, I experience leukorrhea (vaginal discharge) throughout the month.
I was previously diagnosed with iron deficiency and took Fefol for 3 months, but I did not notice significant improvement. I am currently taking Star-D and Becefol.
I would like to ask whether these symptoms could suggest a condition such as endometriosis, adenomyosis, fibroids, PCOS, or any other gynecological issue. Also, what investigations would you recommend (such as pelvic ultrasound, blood tests, or hormone tests)?
