(1). Patient needs chest Xray and echo to rule out pleural and pericardial effusion/pericarditis as the pain is postural and more on bending forward. Physical examination is also very important to look for any added sounds or muffled sounds before proceeding to investigation only then a proper treatment can be advised..
(2). As patient has previous history of cardiomyopathy he needs a fresh echo to look for his LV functions. A 6 min walk test can also be useful to assess his cardiopulmonary functions..
(3). An exercise tolerance teat can be done to rule out any cardiac pain..
(4). Stone of this size in ureter usually passes out spontaneously or with the help of medical expulsion therapy..
