(1). Dear Fatema,at 5 weeks pregnancy with low HCG, pinkish-brown discharge and one-sided pain must be evaluated urgently. Your progesterone injections/suppositories are appropriate, but these new symptoms can indicate threatened miscarriage or ectopic pregnancy. Please do the following: Get a repeat HCG after 48 hours then get an urgent pelvic ultrasound also, Continue progesterone as advised If bleeding increases or pain becomes severe, go to Emergency immediately These symptoms can still settle with proper monitoring, but evaluation should not be delayed. (2). I am sorry that she has to bear pain for long. So normal X-ray is common in this type of injury because it only shows bones. Pain lasting this long is often due to soft-tissue problems (muscles, tendons, bursae)which do not appear on X-ray. Since the pain has continued for 3 years despite physiotherapy and injections, the next best step is an MRI of the right shoulder to check the rotator cuff and other soft tissues. If MRI is normal, the pain may be due to chronic muscle or post-traumatic pain, which usually improves with proper targeted physiotherapy and posture correction, not surgery..

Your symptoms are related to allergic airway irritation or prolonged upper respiratory inflammation that have worsen with smog and dust exposure. Along with ur current medicines, please try to: 1. avoid outdoor exposure during heavy smog 2. use a mask when outside 3. keep urself well hydrated 4. consider steam inhalation once or twice daily for chest comfort 5. Use humudifier at home to balance smog if possible If symptoms like fever, chest tightness, or cough persist after all these measure, or if breathing becomes difficult, its necessary to get chest examination and possibly a chest X-ray to rule out infection or asthma-related issues.
