(3). As the patient is complaining of irregularly heart beat so it is advised to get a holter done if its occuring frequantly as static ECG wont record it. And if its nit occuring frequently then electrophysiological studies are advised to look for any rhythm abnormilities. Once diagnosed then can be treated accordingly..

(4). Patient needs chest Xray and echo to rule out pleural and pericardial effusion/pericarditis as the pain is postural and more on bending forward. Physical examination is also very important to look for any added sounds or muffled sounds before proceeding to investigation only then a proper treatment can be advised..
